|Eligibility:||Homeowners, renters, mobile home owners, residents of nursing homes and shelter care homes are all eligible if they meet the following requirements: 1)65 years old, total household income less than $14,000, or 2) disabled, unable to work for 12 months, and income is less than $6,000.|

|Services Provided:||Medical relief (pharmaceutical card) and/or a property tax grant. The pharmaceutical card is $40 or $80 per year, depending on household income. An additional $15 or $20 is paid every month that a prescription is filled. After receiving $800 worth of medicine, recipient must pay the monthly fee plus an additional 20% for each prescription. Forms are available at banks, senior centers, state offices, and hospital chapels. |Comments:||Reduced fee license plates: circuit breaker tax relief recipients are eligible for a 50% discount on their license plate fees.
