Eligibility:A needs assessment can be completed with anyone 60 and older at no charge. Financial criterion based for the Illinois Department on Aging Community Care Program.

Services Provided:Care Coordination: comprehensive assessment of needs of the frail older adult, referral to services needed, assistance with service linkage, and quality case management
Ombudsman: assists nursing home residents to resolve concerns or complaints
Adult Protective Services: investigates and resolves reported cases of abuse, neglect or financial exploitation for older adults (60 and over) or adults with disabilities (age 18-59)
Money Management Program: assists individuals with bill paying, including writing checks, budgeting, and balancing the checkbook
Caregiver Support Program: offers support services, resources and counseling for caregivers and for older relatives raising children
Support Groups: Grand Families (for grandparents and other relatives raising children) 2nd Friday of the month from 10:00 a.m. to 11:30 a.m. at First Presbyterian Church, Conference Room 1, 400 E. Carroll Street, Macomb; Caregiver Support Group, 3rd Thursday of the month from 1:30 p.m. 3:00 p.m. at 804 W. Jackson St., Macomb
